DCSS requires the following documents to be uploaded to the application:

Student’s Certified Birth Certificate or clear copy
Parent/legal Guardian’s Driver’s License/Photo ID/Passport
Custody/Guardianship/Court documents (if applicable)
Student's social security card or waiver
Proof of Residency documentation – Click here for a list of accepted documents

These documents can be uploaded, but are not required at the time of registration:

Immunization Georgia Form 3231 (marked complete for school attendance)
Vision, Hearing, Dental, and Nutrition (BMI) Georgia Form 3300
Withdrawal form from previous school (if applicable)
Last report card (if applicable)
Discipline history for middle and high school students
Student's transcript for high school students

Examples of Proof Of Residency:

If you rent: A current lease agreement AND a current utility bill (power, water, or gas) or connection letter. The utility bill must be dated within 30 days and not indicate a disconnection. Both the lease agreement AND utility bill must be in the parent/legal guardian's name.

If you own: A current mortgage statement or tax bill, AND a current utility (power, water, or gas) or connection letter. The utility bill must be dated within 30 days and not indicate a disconnection. Both documents must be in the parent/legal guardian's name.

If you live with another family, the Multi-Family Residence Form must be signed by the homeowner or lessee and the enrolling parent/legal guardian. Both signatures MUST be notarized individually. You will need to provide the Multi-Residence Form AND the homeowner or lessee's current lease or mortgage statement AND a current utility bill (power, water, or gas) or connection letter. The utility bill must be dated within 30 days and not indicate a disconnection. Access a Multi-Family Residence Form here.

DCSS reserves the right to request any of these documents at any time. Additional documentation may be needed to complete registration.

Please do not register your student until you are ready for them to start school within 5 days of approval of the application. THIS DOES NOT APPLY TO SUMMER REGISTRATIONS FOR NEXT SCHOOL YEAR.
